Pain spreading above and below the back of the knee; unable to squat.

Approximately five years ago, the patient initially experienced pain around the kneecap. Over time, pain developed above and below the back of the knee, making squatting and kneeling difficult, and accompanied by a heavy, dull ache in the lower back. The patient had no prior medical history. At the initial examination, pain occurred when bending the knee joint beyond 90 degrees, the knee was slightly bent when lying on the back, and a gap was observed between the back of the knee and the bed. There was also severe tightness in the posterior thigh and pain during stretching.

Treatment and progress
Acupuncture was performed on points on the posterior thigh, buttocks, and back. After the treatment, although there was still some tightness in the front of the knee, squatting became easier. In addition, the patient was instructed on stretches for the posterior thigh and buttocks at their request. There have been no further reports on symptoms.

Summary
Acupuncture treatment on the posterior thigh and glutes was effective in treating chronic knee joint pain. Muscle tension in the posterior thigh is thought to be a contributing factor to the limited range of motion in the knee joint. Providing stretching instruction as self-care is expected to help prevent symptom recurrence and improve self-management abilities.
