The patient came to the clinic complaining of pain in the left buttock. The symptoms began five days ago and started occurring while playing tennis. Previously, the patient had felt pain in the right buttock, but now the pain is mainly in the left ischial region, with a strong feeling of stiffness and heaviness, making it impossible to assume stretching positions that stretch the piriformis muscle. The pain is severe during movement, and worsens especially when standing for extended periods. In daily life, it is interfering with tennis movements and is affecting activities to such an extent that it is becoming a burden. The patient has not received any diagnosis or treatment at a medical institution before, and has not experienced any other physical discomfort related to the symptoms.

Treatment and progress
Upon initial examination, palpation revealed significant tension throughout the buttocks, particularly in the piriformis muscle. Because the patient had difficulty assuming a stretching position, acupuncture was performed on related points in the shin and lumbar region to address the buttock stiffness. Finally, a treatment was performed to release tension in the hamstrings. After the first treatment, the buttock stiffness disappeared, the patient was able to assume a stretching position for the piriformis muscle, and expressed satisfaction with the improvement in symptoms. Since then, there have been no reports of symptom recurrence or new symptoms, and the patient's good condition has been maintained.
