I have pain in my shoulder and the top of my foot. The shoulder pain is due to overexertion from playing tennis. The pain is similar to muscle soreness, spreading from the armpit to the ribs, centered around the shoulder joint, and worsening with movement. In particular, internal and external rotation is restricted, and there is a strong feeling of being pulled in the side, making movement difficult. The pain in the top of my foot is felt when stepping or putting weight on it, and it is affecting my gait to the point of limping. These symptoms have become more pronounced since a week ago, after playing a series of tennis matches. I had my foot examined at an orthopedic clinic, but X-rays showed no abnormalities in the bone.

Treatment and progress

Upon initial examination, palpation revealed significant tension around the shoulder joint, particularly on the lateral side from the armpit to the ribs. Strong tension was also observed between the second and third metatarsals on the top of the foot. Acupuncture on points in the lumbar region and hip joint, related to shoulder and foot pain, significantly reduced the pain. Further acupuncture on points in the scapula and back resulted in approximately 9% reduction in pain. Following treatment, shoulder movement pain was eliminated, foot limping ceased, and daily life and sports activities improved; therefore, treatment was concluded.

Summary

This case involved simultaneous shoulder and foot pain, likely caused by overexertion from tennis. Acupuncture focused on acupoints in the lower back and hip joint, which are related to the shoulder and foot pain, resulting in significant symptom improvement. Treatment of acupoints in the shoulder blade and back completed the treatment, almost completely eliminating the pain. This case demonstrates that in cases of pain caused by sports-related overexertion, an approach that addresses not only the local area but also related areas is effective.
